Learner Documents:

• Birth certificate (if non South African, please provide proof of permit, e.g. asylum/ refugee permits)
• Clinic Card (page with immunizations history
• Progress report if learner attends creche/preschool

Family Documents:

• Proof of income for all adults: payslip/letter from employer, not older than 3 months
• Proof of unemployment for all adults in house: affidavit
• Proof of grant: print from SASSA’s office or SASSA account statement
• Bank statement of parents/guardians: 3-month statement
• ID of parents/guardians (if non South African, please provide proof of permit, e.g. asylum/ refugee permits)
• Proof of address: water or rates bill / account statement – not older than 3 months
• If foster parent or legal guardian: legal placement/adoption papers
